Graduate programs at HU are designed for working professionals focused on career advancements who can apply what is learned back to their workplace. To integrate professional work experience into all phases of the Graduate degree programs, all students must submit an Academic Work Placement Form or the Work Placement Waiver/Experience Form. An F1 student seeking Curricular Practical Training (CPT) authorization, must have an approved Academic Work Placement Form from Records and Registration prior to requesting CPT authorization from the International Student Office (ISO).

 

How do I know which form to submit?

  • Complete the Academic Work Placement Form if you have a current or active work placement. 
    • Please include a copy of your official offer letter or employment letter, which includes: employer's name, full address, and employee's start date. 
    • Also include a copy of your job description letter that highlights all your job duties/responsibilities. 
  • Complete the Work Placement Waiver/Experience Form if you do not have a work placement.

  • After registration opens (New Students);
  • If your active job/training/position changes;
  • If you gain new employment during your studies; 
  • If you change programs or educational level; or,
  • If you want to extend your CPT authorization.

 

How do I access and submit the Form?

Download and complete the form attached below. Submit the form to the Records and Registration Support Portal by creating a New Support Ticket and attaching the form(s) to it. 

 

Work Placement Waiver/Experience Form

 

When do I complete the Waiver/Experience Form?

  • After registration opens (New Students);
  • If there is a loss of work placement during your studies; 
  • If you change your education level; or
  • To document work experience.

 

How do I access and submit the Waiver/Experience Form?

Download and complete the Waiver/Experience Form attached below. Submit the form to the Records and Registration Support Portal by creating a New Support Ticket and attaching the form(s) to it. 

 

What if I am on an F1 Visa?

Students on F1 Status must have work authorization for employment in the United States. Students need CPT authorized on their I-20 before they can begin to work. For more information about CPT Work Authorization, click HERE.

  • If you are a student on an F1 Status seeking CPT authorization, you must submit your approved Academic Work Placement Form to ISO for CPT authorization approval HERE
  • If you are a student on an F1 Status and are not seeking CPT authorization, you must submit the Waiver of Academic Work Placement Form to Records and Registration. Once the waiver is approved by Records and Registration, no additional follow-up with ISO is required.
